Form below is for the type of industry subjected to FIRST SCHEDULE (refer to the new regulation). If the premises subjected to the First Schedule, form below must be submitted annually. Please refer to Regulation 18.

Meanwhile, form below is the Written Declaration on Design and Construction of Air Pollutin Control System, refer to Regulation 7(5) within thirty days after the commencement of operations at the premises.
